About the patterns
Scalpel handles here are sized specifically for the small blades used in corneal and cataract work, not general-purpose surgical blades. A blade breaker and holder lets the surgeon snap a fragment (razor-style) blade to a precise, controlled cutting edge, and then holds that fragment securely for use - the Troutman pattern is the smaller of the two, Castroviejo the larger.
